Core SAAFD training
Learn the six phases, their guiding questions, and how to connect traditional quality tools with a deeper investigation sequence.
Education & implementation
Practical training programs are in development for people who investigate failures, lead improvement efforts, and build stronger systems.
The SAAFD education program is being developed around the methodology, its supporting quality tools, and hands-on cases. Planned learning experiences will connect concepts with evidence gathering, teamwork, decision making, and prevention design.
Program dates, formats, prices, and enrollment information have not yet been announced. You can request updates or discuss a future organization-focused session today.

Learn the six phases, their guiding questions, and how to connect traditional quality tools with a deeper investigation sequence.
Apply the method to challenging, cross-functional cases with uncertainty, interacting pathways, and difficult prevention decisions.
Build facilitator skills, guide team thinking, evaluate evidence, and support consistent application using planned instructor resources.
Explore how better failure investigation and prevention practices can influence decisions, organizational learning, and improvement culture.
Discuss how the method might fit existing CAPA, FMEA, quality, engineering, service, or operations systems.
Companion learning materials and application aids are planned to help reinforce the method beyond a workshop.
The learning philosophy
Training will emphasize what people do when they encounter uncertainty: ask clearer questions, challenge easy answers, test the mechanism safely, and make prevention part of the system.
Where AI is included, it is positioned as an aid for organizing information, identifying alternative lines of inquiry, and challenging assumptions. Human experts remain accountable for evidence, safety, validation, and final decisions.
